The officer comes in without a light. The suspect fires first and the officer returned fire. The suspect later died from a self-inflicted gunshot wound and the suspect’s mother suffered a grazing wound. How can we expect officers to make sound decisions in a gunfight when they can’t see the threat? We teach officers that they’re accountable for every round they fire. Firing into darkness makes that responsibility impossible. Whether it was the officer’s decision or the agency’s decision not to equip the handgun with a weapon-mounted light, it was a mistake. Backstory: Youngstown Police say an officer responding to a domestic disturbance shortly after midnight on June 13, 2024 encountered O’Malley inside the home with a woman and her daughter. As the door opened, the daughter reportedly told the officer O’Malley had a gun pointed at her mother. According to police, O’Malley fired first and the officer returned fire. O’Malley was found in a bedroom with a handgun nearby. Officers and EMS provided aid. The woman suffered a grazing wound to her thigh; no officers were injured. The Mahoning County Coroner’s Office ruled O’Malley’s death a suicide. An autopsy found that he died from a contact-range gunshot wound to the head, fired from his own handgun. He was pronounced dead on June 13.

OFFICER INVOLVED SHOOTING WHEN SUSPECT REACHES TOWARD HANGUN 1. A Taser is an option but never a guarantee. Officers must always be prepared with a backup plan including lethal-force coverage when circumstances warrant it. 2. Law-enforcement encounters are dynamic, and crossfire can develop in seconds. Every officer is responsible for recognizing and avoiding it. That begins with properly positioning yourself during the initial contact and continually adjusting as the incident unfolds based on the locations of your fellow officers. Before firing, officers must know their target and backdrop, move to a safer angle when possible, or hold their fire until they have a clear and responsible shot. Backstory: On September 15, 2026, Alliance Police responded to Wally Armour’s car dealership after 67-year-old Joseph Derrico Jr. allegedly argued with employees and displayed a handgun toward one of them. Officers located Derrico inside his vehicle a short time later on Johnson Avenue. They repeatedly ordered him to turn off the vehicle and place his hands on the roof. Police said Derrico refused to comply. An officer deployed a Taser, but it did not incapacitate him. The newly released body-camera footage shows the officers opening fire as Derrico reached toward a handgun. Derrico was pronounced dead at the scene. A firearm was recovered, and no officers were injured.
